Sure Seal Restorations LLC may allow you use of a company vehicle/trailer/ or equipment at your request to conduct business for Sure Seal Contractors ONLY. During the duration of said vehicle/trailer/ or equipment use, you will be solely responsible for the condition in which the rented items are kept, normal everyday wear and tear is to be expected. The subcontractor in use of the vehicle/trailer/ or equipment will be responsible for all routine maintenance and keeping the rented equipment/trailer/ and/or vehicle damage free. The vehicle/trailer/ or equipment must be cleaned thoroughly to the condition it was given and the gas tank must be returned to the level it was given. If the items listed above are not completed, employees will be financially responsible for any amount used to cover these services. If the renter is unable to make the payment required to repair any necessary damage, Sure Seal Contractors will compensate for the repairs and the renter will be billed for the cost of repairs plus 10%.

*Required to have a valid Indiana driver’s license

*General Liability insurance provided by SSR. If your license requires any additional insurance, it is your responsibility to provide payment for additional insurance to SSR.
 

By signing below, you knowingly acknowledge the terms in which this vehicle/trailer/equipment is provided to you.

SURE SEAL CONTRACTORS RESERVES THE RIGHT TO REVOKE RENTAL AGREEMENT AND REPOSSESS THE VEHICLE/TRAILER/OR OTHER EQUPITMENT AT ANYTIME FOR ANY REASON
